Toolbar--blank in my window, but fully customizable!

I can't figure this out.

I have created a toolbar programmatically and everything seems to be in
order. There's just one small problem: the toolbar is blank in my window! It
seems to allocate the right amount of space for it if things were working,
but you don't see a thing. So I thought maybe my toolbaritems aren't
registering, but no, if you go to customize the toolbar, it has everything
there as it should be and it says I'm working off of the default toolbar.

Any ideas?
