focus follows cursor/clickthrough again

I know this has come up recently but looking through a few dozen archive posts hasn't been illuminating so
I gave up and came here.

As far as I know the Apple window manager doesn't support "cursor follows focus" of "sloppy pointers" (new term to me)
for all the right reasons. Nevertheless there are those who want it badly enough to work for it.

Is there a realistically practical path to implement that behavior. Can I say to someone "if you want to do that
start here."
