In the event of a future FAQ, this is one of the questions that should be
answered...

Apple has not made the menu extra API public (just as they haven't made the
dockling/dock extra API public). However, it has been reverse engineered.
You can check out ASM (search the archives or versiontracker) - which is
open source, and demonstrates how to do the same.

And no: it cannot be done in carbon.
