Where Does Application Name Come From?

Somewhere in the middle of my project, I decided to change its
name. I changed all the occurrences I could find in the "target"
panes of the Project Builder, and changed the menu title for the
main menu in the nib file. Nevertheless, the name that appears on
the title bar is the old name, and it also appears to be looking for
a help file under that name from the default application help action.

I have cleared my preferences, done several clean builds, and even
run a copy of the app as a different user. I have grubbed through
every file in the build directory (as well as my source code, of
course) looking for strings containing the old name and cannot find
any. I am stumped. Short of starting a new project with the
correct name (if possible) and copying all the source code into
it, what else might I look at?
