Yea, this is so confusing. my experience is with relational databases, mostly db2. In the 'old' procedural world.
I have converted my thinking that it is necessary to use objects and then map to xml and/or relational tables. The objects you create yourself, then map them to the relational tables.
You need to forget some of what you know. I plan to get DB2 working on the Mac, but at present it is not available. DB2 has some new XML features that map XML to relational tables.
I have Apache Xerces working in Cocoa, (sort of, working on some problems with some of the interfaces), I can map Objective-C objects to XML elements and back again.
