Application Icon...

I'm working my way through the Garfinkel & Mahoney book from O'Reilly and I've hit a spot that has me baffled...

I'm in Chapter 13 about saving NSDocuments. The saving works just fine but the icon which I put into the App earlier in chapter 13 didn't appear with the files in the finder.

Also, the MathPaper application was not "bound" to the filetype (extension ".matp") until I manually changed it in the Finder... After changing it in the finder, the icon changed, but only for that document's icon (even though I chose to "Change All"). The other documents had the right app associated with them, but the icon didn't change.

Did I do something wrong? I used the name "PaperIcon" and "PaperIcon.icns" in the "Icon file" box (the text of the book said one thing, the figure showed another), but the results were the same each way.
