Reliable way to add Application Icon

I have added application icons to two PB projects, one worked after a while, the other refuses to work and always gets a generic application icon. Here are the things I did that worked once but not twice:

1. Created icon in Photoshop
2. Created "AppIcon.icns" file for that image using Icon Composer
3. Added AppIcon.icns to my project
4. Entered "AppIcon.icns" in the application settings for the target.
5. Cleaned the target
6. Rebuilt everything
7. Log in and out
8. Changed version number
9. Used terminal to browse the application bundle and verified that "AppIcon.icns" is in the bundle and that the plist file points to that file

I can not think of anything else. Is there a reliable process to install an icon and make it appear for that application?
