I thought I'd summarize the responses:
(1) iPhoto uses pictures for its custom controls. This allows them to (most likely) use standard Cocoa controls to do the work (for example, the Square Button can be un-bordered, and with a couple of pictures, can easily look like anything you'd like). So, just draw what you want, and use it.

(2) To make the draw-what-you-want part easier, Sherm Pendley recommended a program named AquaTint, which can take arbitrary shapes and make them look like an Aqua button. This is on versiontracker, or at <http://www.sticksoftware.com/software/Aquatint.html>. It can generate Aqua blue and Aqua gray output, as well as pressed, not-pressed, and disabled. It seems to work quite well.
