When I got that particular message, I was told it was because I had tried to enter the wrong password three times, or something like that. In other words, it was a security precaution on their side, easily cleared up.

*That's* where that form was. I mentioned it earlier but couldn't find the URL. My bookmark points directly to bugreport.apple.com rather than the page mentioned above. The same may be true for Ondra.
There should be a link to the Bug-Reporter-bug-reporter on the Bug Reporter page itself. I'll send a message to this effect to email@hidden.
