Is that documented truth or a good guess?
I'm not trying to be provocative, but I would like to know if this is a bug (that could be fixed in a future revision of Mac OS X) or a deliberate design choice by Apple.

That's because toolbar items are, themselves, shortcuts. Every toolbar item should have an equivalent menu item, which is where you should attach your keyboard shortcuts.
