It's more or less documented in Menus.h. There should be a sample Carbon
project on the sample code page at Apple.
Also, search the Carbon list for "Contextual Menu Workshop". You should
find some links to a guy who has a bunch of free sample CMM source you
can tinker with.
If you figure out how to write one in Cocoa, let me know...
