I used Icn Builder and made four versions of the icon, from 16x16 to 128x128. The only thing I could think to do was include the icn file in PB and Interface Builder. It did not work and there is nothing in help.

You only need to include the file in PB, not IB.

Once you've included it, you set the application icon by opening up the target settings, and click on "Application Icon" under "Info.plist Entries" and type it in here, complete with the .icns extension. Then rebuild your project. If it isn't appearing immediately in the Finder, you may need to quit and relaunch the Finder for it to notice...
