Download either Iconic ( <--shameless plug) or Pic2Icon and drop your
photoshop file on it. It will make the icns file for you.
Make sure you have the background layer in Photoshop turned off when
you save it (i.e. you should be able to see the background grid behind
your icon in photoshop).
