One more data point, if the screensaver is active, I can't use the
procedure to logout. I'm running 10.2.6.

Well, not really. The Log Out menu item is just a normal menu item.
If you open the Apple menu and hold down the Option key, you'll see
that the "Log Out..." menu item changes to "Log Out", indicating that
the option key makes you log out immediately without a warning >> dialog.
True, but since the AppleMenu is disabled you are unable to do this.
My point was this: In order to disable these key commands you have to
add the key equivalents to your app. If you do not have
command-shift-q or command-shift-option-q in your menu items, it will
default to the systems logout commands.
