Someone moves a mouse all day, they get RSI of the wrist/forearm. They move their arms around all day (Minority Report style), they get shoulder/neck/back problems.

How would you describe the effects of trackpads and trackballs in that context?

What do you mean? I use trackballs (Kensington ExpertMouse) exclusively, and I got mild tendonites after a particularly severe month. They for most people is change, I think (personal theory here) - you get twingy on a mouse and you switch to a trackball, things get better - primarily because you've remove the repetitive motion (you've set up a _new_ set of repetitive motions, but things take a while to kick in - and you can learn better habits easier with new motions than old ones).
