application not quitting

I've got an application that doesn't quit when you select Quit. If I
set a break point on terminate:, the breakpoint is hit. However, my
application delegate's applicationShouldTerminate: never gets called
and the app never quits.
Any ideas what could cause this or how to figure out what the problem is?
