My personal recommendation is Eric Raymond's "How To Ask Questions The Smart Way":

It addresses many of the issues that arise on this list. There are a couple of sections that are largely irrelevant, however these are easily identified and skimmed (e.g. "Web and IRC forums..."). **Note also the section on "How To Answer Questions in a Helpful Way"**.
