Making the correct button the default button?

In NSRunAlertPanel() and its relatives, it appears to be impossible to control which button is the default button, i.e. the design of NSRunAlertPanel() et al appears to assume that the default button is always the right most button. Or am I missing something?

For example, I'd like to be able to have this layout with the Cancel button as the default.

Do you really want to end it all?

(Don't End) (Cancel) (End It)
