It's important to consider who the end users will be. I've seen many an
occasion where a user will dismiss a dialog (often by pressing Return
or Enter), cause a significant loss of data, then tell me that they
didn't notice what was written in the dialog box (and this is in
Apple's programs). I realize that you can't protect everyone from
themselves, but the extra 0.7 seconds it takes for a power user to have
to click a button is nothing compared to the pain experienced by some
non-power users. Most users are not so irked by an extra click as we
"experts" might be.
I highly recommend offering a way out, and to follow Apple's guidelines
which state that the default button should not cause any significant
loss.

You mean like the one Finder (X.3.2) presents when you try to
copy/move an item into a folder that already has an item with the same
name, then one with a Replace button and a Stop button, and NO DEFAULT
button?
I would prefer the default to be the "do what I just asked you to do"
button: if I'm using a computer and tell it to perform some action
it's almost exclusively because I wanted it to perform some action.
I agree >100% on this point. The computer is my tool, it's not its
job to second guess me.
