You certainly didn't read it in the HIG:
"Toolbars are useful for giving users immediate access to the most
frequently used commands. Any item in a toolbar should also be
available as a menu command." - Control Behavior and Appearance,
section Toolbars.
Nope, it was a discussion with an engineer, and it was in fact the
opposite.. that not all menu items should be made available in a
toolbar just because they apply (i.e. no Save in the toolbar as an
example).
