Regarding Q2 - I've had this occur and fixed it by opening
/Users/<user>/Library/Preferences/com.apple.Xcode.plist in Property
List Editor. Locate the entry 'PBXHelpWindowContentConfiguration' and
delete it. Then run up Xcode and it will be back to the default layout
settings for the help window. If your concerned about editing this make
a backup of the file first!
