It's not necessarily a problem. Not everything is necessarily connected, and it doesn't mean there is a problem.

If you want to see what isn't connected, click the list view icon just above the vertical scroll bar in the nib window (the one with the Instances, Clases, etc. tab view). Looking at a new application nib file, for example, the Preferences and Services menu items under the NewApplication menu are unconnected. It's something you might want to be aware of if you're expecting those items to do something in particular, but it's not going to cause any problems in and of itself.
