It says update because installer leaves a receipt in /Library/Receipt. Just delete that file and the button will return to Install.

Not sure about this, but I think installer ignores everything but this file. So deleting the other files is unnecessary, although it may not overwrite files if you don't have that option set so deleting everything is still safer.

I'm testing an install made with PackageMaker. During the install, one of the buttons says 'Update' instead of 'Install'. I'm assuming it's because I didn't delete something I should have.

I removed the application from the applications folder. I removed a frame it uses from /Library/Frameworks. I removed the .plist file in the user/Library/Preferences folder. Is there another file stuffed away in the system that also has to be removed?
