FormMate is a Mac OS X / Cocoa application for filling out a pdf form on your computer.

This seems to be a little misleading, as there seems to be no interaction whatsoever with actual PDF forms. Instead it is a drawing program that can import PDFs and use them as a background. I.e.: Sketch, located in the examples folder.
