Application fails to come to front / show menu bar in Xcode

All of a sudden, when I run my application from within Xcode, it fails to come to the front and the menu bar is never displayed. The window will become the main + key window OK, but the current menu bar remains unchanged.

I'm not aware of changing any application or Xcode settings that would affect this.

Any ideas?
