The instructions near the bottom of that URL are based on some code I wrote
a couple of years ago to implement headers and footers in printed output. I
have since enhanced the code so that the headers and footers print correctly
even when the user has changed the magnification level from the default
100%.
I get asked for this code all the time, but I haven't yet figured out a
convenient way to publish it. It's too long for the mailing list. I'll send
it to you privately, and within the next couple of weeks I'll try to publish
it and post the URL here.
