How are you adding the icon? You're starting with a fresh .icns file,
and not pasting your icons into one copied from another app? I
suppose this is a Cocoa app, right? You wouldn't happen to
accidentally have provided some Carbon resources (like a 'plst'
resource, and 'icns' resources) for your app?
Also, have you made sure your Bundle Identifier is unique and that
you haven't specified an already-used four-character creator code?
Have you tried logging out and back in? After long times of running
without a restart, Icon Services seems to sometimes get out of whack
and mix-and match bits and pieces of icons.
And Finally, you aren't perhaps messing with NSApplication's dock tile image?
