You can use the Carbon Process Manager to find all running apps.
(There may be a Cocoa equivalent that I'm unaware of.)
The Accessibility Manager can find the windows of these apps, but it
requires that the user explicitly enable it. There are SPIs that can
do this as well (Quartz Debug does it, for example) but I don't know
the specifics. If you were determined, and don't mind your app
failing once 10.5 comes out, you could reverse-engineer Quartz Debug.
