I think it's boot. And I think there was discussion from an Apple engineer
that they intended to add periodic purging of files that had been closed and
not touched for some long period (3 days?), so that on long-running systems
the directory wouldn't clog up with orphaned files, but that apps shouldn't
count on stuff remaining in there indefinitely if not actively used.

Anyway, although /tmp is world-writable, /tmp/folders.xxx is not, nor is
/tmp/folders.xxx/TemporaryItems. At least on Tiger. I haven't checked older
systems.
