Default Nib on Document based apps

How does cocoa determine what the default nib to load on document based
apps is?  By messing with PB I noticed that there are extensions you can
associate with a class.  Furthermore how do you know that your
nib_loaded message will be invoked?  I was looking for the documentation
on this and if any1 has a link handy that will serve.
