NSToolbar sometimes does not appear

I have a very bizarre problem, which is that my NSToolbar sometimes
does appear when the application loads, and sometimes does not.
After trying a million things to get it working again, and giving up,
it will eventually start coming back again. Then, later in the day,
it will stop again, and so forth.
Anybody seen something like this before?
