To answer my own question (for the archives):
In Tiger, it seems that whether the Keychain panels display a generic
document icon or the application icon is dependent on where the
application is located (!).
To see this for yourself,
1) Quit the Mail application if it is running.
2) Make sure your Keychain is locked.
3) Drag the Mail application to your desktop
4) Launch Mail. When Mail tries to access the keychain, the "Unlock
Keychain" panel will appear, and it will have a generic document icon
composited on the padlock icon.
5) Cancel the "Unlock Keychain" panel and quit Mail.
6) Move Mail back to /Applications
7) Launch Mail. This time when the "Unlock Keychain" appears, it will
have the Mail application icon composited on the padlock icon.
Weird.
