Multiple keyboard shortcuts

I'd like a menu item to have multiple keyboard shortcuts. In particular, the main shortcut is "Command-+" but I'd like for command- = to work also (i.e. the user doesn't have to hold down shift) ... just like in Apple's "Preview".

How do I do that? And how do I handle international keyboards?
