I do not think that the API for direct plug-ins to iSync is public at the moment. I contacted developer support a couple of years ago and at the time they were not making that API available; the subsequent introduction of the Sync Services API makes me think that they are not too keen to have people add their icons to iSync and it almost seems like Apple are aiming to deprecate iSync.
