install app missing??

I had a user write to me the other day and say that she had trouble
installing my app (http://www.ziggurat.ch/products/adsanity/). She
said the "pkg" file type was unknown to her freshly installed Tiger.
I asked her to look in the Applications/Utilities folder and, sure
enough, she had no Installer.app file. She grabbed a copy of
Installer from a backup and managed to get it working. But it got
me wondering how she managed to not have it in the first place. Is
there a way to install Tiger without it that I'm not aware of? I
can't see much of a reason for that. But if there is, I would like
to know so I can adjust my software. Of should I just chalk this up
to a strange user-side error?
