This code sample ended up fixing the problem for me. There is actually a bit of glue code that is needed to get it to work, included in this sample.

You need MachOFunctionPointerForCFMFunctionPointer, from the CFM_MachO_CFM sample code:

Your Mach-O code will need to create some glue code around the CFM
callbacks, and call them through that.
