HexInputServer not working

I built the latest (file created July 2004) Apple example code for a
Cocoa text input method, HexInputServer, two ways, using Xcode and using
the terminal (following the read-me instructions, only substituting
"xcodebuild install" for "pbxbuild install").

It builds both ways with no problems, but the resulting bundle, when put
in the "~/Library/InputManagers" directory and after re-starting does not
appear as a choice in the International preferences panel, nor does the
"Edit/Input/Hexadecimal Input" menu item in TextEdit stick when you
select it. In short, the HexInputServer project cannot be used as a
template for creating a Cocoa text input method.

What gives?
