If iTunes were any sort of "pure" Cocoa, it would be accessible via Voice Over. There is virtually no accessibility with Voice Over. I think iTunes for OS X and Windows are very specialized applications. I don't know if Apple has a Cocoa for Windows API, but even if they do you can be damn sure it won't be released any time soon.
