I trudged through my code again, and found that I was calling
initWithWindowNibName in the init message in my Application object.
When I moved this to the applicationDidFinishLaunching message the
menu disappeared.
I still don't understand why calling initWithWindowNibName in the
init method would have added this menu?

Are you opening any windows prior to applicationDidFinishLaunching
being called?
