-> Make sure they're .icns files and not .tiff or any other image format.
-> Make sure your icon has been added as a bundle resource to the built application.
-> Make sure you correctly typed the name (also watch out for upper/ lowercase, or your app'll break on case-sensitive HFS or UFS like Photoshop does). Don't include the suffix. Your icon should show in that example box next to the icon file name entry field. (for an app icon)
-> Log out and back in or duplicate your app's bundle in the Finder to get Icon Services to reload your app's icons
