Err... you open "My file with / in it.txt" in a Cocoa application :)
Carbon apps show "/". Cocoa apps show ":". It's a little bonkers.

how do I programatically create a NSString holding a path with a file name that contains a "/" character?

/Users/monk/Desktop/My file with / in it.txt

where "My file with / in it.txt" is the name of the file.
