What i finally decided to do was to add only 1 toolbar item and send
setView: to it with a custom view that implements all the controls i
need. It's really not much more work. Action handling is a bit
different but that's really a minor issue.

I don't think that will help Alex. He actually wants to achieve
iTunes-like toolbar behaviour, but with unified titlebar and toolbar
look.
It might be possible if you implemented your own spacer items, rather
than using the standard flexible spacer.
