Is it possible to get notified when the user clicks on the window's field editor? There seems to be no obvious way from the NSText API.

You can certainly be notified when the selection changes, via NSTextViewDidChangeSelectionNotification. You can find out when editing begins or ends. There's no general notification of arbitrary clicks.
