I think there is just a bug in 10.4.x with NSViews not refreshing properly.
About once a day I'll press return in Xcode and the lines below the
current line don't shift downwards. Or I'll type an open-quote, all the
text below the current line will change color, and then I'll close-quote
but the text color doesn't change back. Oh, and this morning all the
buttons in my Calculator window disappeared at one point :)
So (to bring this back on-topic) this bug might not be a
toolbar-problem; it could just be a general NSView refresh bug. I
thought I had a radar filed on this but I no longer see it.
