The key "NSFontNameAttribute" should be set to an NSString instance
(the font name), not an NSFont instance.
So, for the first attribute key, instead of:
[NSFont fontWithName: @"Monaco" size: 12.0], NSFontNameAttribute,
use:
@"Monaco", NSFontNameAttribute,
