OK,
I'm done with this tutorial, but I'm still missing the point:
My application does not need to create any new documents.
It needs to open an existing document upon startup and allow user to
make changes to it without being asked for a file name to save
changes to.
What am I missing?

Given your description, it seems you're simply missing the
