10.4.9 stangeness?

Is anyone seeing strangness with 10.4.9 and initWithWindowNib? My window is being created form a bundle. It is OK the first time. The second time through i get 2007-03-19 13:08:57.698 Adobe Photoshop CS3 [6449] *** -[NSCFArray addObject:]: attempt to insert nil
2007-03-19 13:08:57.698 Adobe Photoshop CS3[6449] *** Uncaught exception: <NSInvalidArgumentException> *** -[NSCFArray addObject:]: attempt to insert nil.


This is in a photoshop plugin that calls uses a cocoa bundle from a carbon plugin. My progress dialogs, before my main window, work fine. It appears to be crashing when trying to add the window to vector and the window is nil.
