Take a look at http://developer.apple.com/technotes/tn/tn2016.html,
particularly kVisualPluginConfigureMessage.  You can pop up a
Cocoa/Carbon window through that.

(Incidentally - sweet baby jesus, they updated TN2016!  Why did noone
tell me about that?)
