Usually I only see this when ether the debug file format is set to stabs or the optimization level isn't zero.

DEBUG_INFORMATION_FORMAT = dwarf
DEBUG_INFORMATION_FORMAT = dwarf-with-dsym

GCC_OPTIMIZATION_LEVEL = 0
