VoiceOver uses the Accessibility APIs to do this. If you have the developer tools installed run the "Accessibility Inspector" app (in </ Developer/Application/Utilities/Accessibility Tools/>) and watch the info in the utility window change as you move the mouse over different parts of the screen. Note that Accessibility has to be turned on (via the "Universal Access" system preference panel) for this to work.
